However, many hospital nurses have little information from regional supporters to provide sufficient transitional care for cancer patients. We intended to elucidate whether reports of patients at home affect transitional care by hospital nurses for cancer patients. After receiving reports about patients at home, 34.5% of hospital nurses felt they should change their transitional care and came to provide care with a vision of the patient living at home. Neither hospital nurses nor home visiting nurses recognized the necessity of direct cooperation because they felt the regional medical liaison office in the hospital acts as a link between them.
