| 内容記述 | 2015年10月には保健師助産師看護師法（以下保助看法）が改正され、指定研修期間における研修を受けた看護師であれば、医師の手順書に基づいて特定行為ができるようになった。この新制度に先立ち、一般社団法人日本ＮＰ教育大学院協議会認定診療看護師（Nurse Practitioner：以下ＮＰと称する）教育が2008年に開始された。ＮＰは「一般社団法人日本ＮＰ教育大学院協議会が認める大学院ＮＰ教育課程を修了し、本協議会が実施するＮＰ資格認定試験に合格した者で、医師の包括的指示のもとに保助看法が定める特定行為を実施することができる看護師」と定義している。ＮＰの勤務する病院には、医師不足を抱える施設もあり、ＮＰは多忙な医師と協働しながら活動を行っている。特に、医師は外来や検査、手術など重複する業務が多く、病棟に常駐することは難しい。このため、ＮＰが、医師不在の病棟で看護師などの医療スタッフと共に効果的に活動することが期待されている。そこで、どのような場面でＮＰが特定行為を実施することが求められるかを明らかにすることを目的とし、ＮＰが在籍する₂施設、839名の中から単純無作為抽出法で合計300名の看護師を抽出し、ＮＰに関する看護師の意識調査を行い、210名から回答を得た。38特定行為において、現在ＮＰが実施している特定行為の中で最も多かった看護師の回答は「中心静脈カテーテルの抜去」11.6％であった。今後ＮＰに実施してほしい特定行為の中で看護師の回答が最も多かったのは「中心静脈カテーテルの抜去」42.4％、次いで「褥瘡又は慢性創傷の治療における血流のない壊死組織の除去」38.7％であった。 | |||||||||||||

| 内容記述 | When Act on Public Health Nurses, Midwives and Nurses （below Hossing And Nursing Law） was revised and was the nurse who took a course in a designation training period in October, 2015, a specific act could do now based on doctor's procedure manual. General corporate judicial person Japanese NP educational graduate school conference authorization medical examination and treatment nurse （Nurse Practitioner: is called NP in the following.） education has been begun in 2008 before this new system. NP defines it as "the nurse who can be the person who finished the graduate school NP curriculum a general corporate judicial person Japanese NP educational graduate school conference admits and passed the NP certification test this conference puts into effect and put the specific act Hossing And Nursing Law sets under doctor's comprehensive directions into effect". There are the facilities with a doctor shortage in the working hospital in NP, and NP is active while collaborating with a busy doctor. Particularly, as for the doctor, it is difficult outpatient department and for inspection, a lot of duties to repeat including the operation to be resident in a ward. Therefore, it is hoped that NP is active with the medical staff such as nurses effectively in a ward of the doctor absence. However, there are few studies that clarified the recognition of the nurse for NP working in a hospital. Therefore it is medical treatment nurse （Nurse Practitioner in what kind of scene: It was intended to clarify whether NP） was demanded and extracted 300 nurses in total by a simplicity random sampling method from 2 facilities, 839 people whom NP was registered at and performed the attitude survey of the nurse about NP and got an answer from 210. In 38 identification acts, the answer of the nurse whom there was the most in the specific act that NP carried out was 11.6% of "withdrawals of the central veins catheter" now. Nurse's answer followed that it was most "withdrawals of the central veins catheter" 42.4% and was 38.7% of " the removal of the sphacelus which there was not of the bloodstream in the treatment of a bedsore or the chronicity wound" in the specific act I'll want to put into effect in NP from now on. |
