近年、看護師の役割拡大の一端として診療看護師（NP）（以下、NP）の活動が注目されている。国内外の心臓血管外科領域におけるナースプラクティショナーの活動について、文献的検討を行い、また筆者が実際におこなったNP活動を踏まえて、今後の展望を検討した。国外報告では比較研究が主だったのに対し、国内報告では主に自施設での活動内容の報告が多く、主な効果としては医師の労働時間の短縮と業務負担の軽減、入院日数の短縮などであった。著者が自施設にて行った活動についても既報告と同様の活動が可能であり、医師・看護師等の協力や指導を得ることにより、多岐にわたる活動を支障なく行い得、患者の安全性の向上、医療職の労働環境の改善に寄与できるのではないかと考えられた。我が国での報告はまだ少数で、自らの活動をエビデンスとして積み重ねていくことは重要であり、日本版NPの制度化にむけても寄与できるのではないかと考えられた。 |

In recent years, nurse practitioner（NP）activities have been involved in expanding the role of nurses. In this paper, studies focusing on NP activities in the field of cardiovascular surgery in Japan and overseas were reviewed, and future prospects were examined based on the actual NP activities performed by the author. In contrast to foreign studies, which were mainly comparative studies, domestic studies primarily reported activities at their own facilities, and the major effects were reduced physicians' working hours and workload and shorter hospitalization stays. The activities performed by the author at his own facility were similar to those already reported, and several activities could be performed without hindrance with the cooperation and guidance of doctors and nurses, which helped improve the patient’s safety and medical professionals’ working environment. Only a few studies have reported about NPs in Japan; therefore, further assessment of their activities is needed, which can contribute to the institutionalization of the Japanese version of NPs. |
